Healthcare Excellence for Government Heroes

Agrahara insurance scheme was established by National Insurance Trust Fund Board Act No 28 of 2006 with the Ministry of National Policies & Economic Affairs, to uplift the living standards of government employees and their families island-wide. Nearly 700,000 government employees and their dependents are entitled to premier healthcare services from private hospitals under Agrahara insurance scheme. Driven by the strong sense of corporate social responsibility, Lanka Hospitals has partnered with the NITF since 2010 and has continued to offer superior healthcare services leaving no additional burdens or anxieties on individuals attached to the public sector.

How To Apply

For Whom?

Married Employees

Member, Spouse and Children (only if they are unmarried, unemployed and below 21 years old)

Unmarried Employees

Member, Parents (only if the parents are below 70 years old)

Required Documents to Claim Agrahara

Submission Deadline: Claims must be forwarded to the National Insurance Trust Fund within 90 days of discharge.

If Both Husband and Wife are Agrahara Members

The patient member should submit the claim form.
Claims for children should be made by one member only.

Required Documents for Dependents (Married)

For Spouse: Certified copy of the marriage certificate.
For Children: Certified copy of the birth certificate.
For member’s Mother and Father: Certified photocopy of birth certificate and NIC of the insured person

Required Documents for Dependent Parents (Unmarried)

Certified photocopy of NIC of Mother / Father.
Letter from the Head-of-Department confirming unmarried status.
A certificate from Grama Sewa Niladari counter signed by the Divisional Secretary to the affect that Mother/Father depend on the insured person

Documents for All Claims

Original or certified copy of the diagnosis card (including Name, Date of admission, Date of discharge, Signature and stamp of the treating Medical Officer).
Original receipt of advance payment (for private hospitals), other paid receipts, and the final bill.
Completed claim form with the member’s signature.
⁠Other Insurance Benefits
If other insurance benefits have been obtained or are expected to be obtained for the same claim, submit certified copies of bills/receipts and a letter stating the amount paid by the other institution.
